Registration
You can sign up either online or by downloading the registration form and mailing
it in.
Online
You can sign up online using the FirstGiving site for PHA which you can
find here:
http://www.firstgiving.com/phassociation
Once there follow these steps:
- Click on "Get Started"
- Click on the "1st Annual Scramble For A Cure Golf Tournament", which as of this
writing is the 11th item on the list which means you have to move to the second
page in the list of events.
- Select "Register online now" and click "Continue"
- Sign up! Make sure you sign up for the Ball Drop too.
By Mail
To register by mail you can download our brochure below, which contains the registration
form. Feel free to print out multiple brochures and bring them to
your friends.
